Few things announce themselves quite like plantar fasciitis. You swing your legs out of bed, put your foot down, and a sharp pain shoots through your heel with those first few steps. It eases as you get moving, then creeps back after you've been on your feet a while. If that sounds familiar, you're in very common company — and there's a clear path out of it.
What is it?
The plantar fascia is a tough, fan-shaped sheet of connective tissue that runs along the sole of your foot, from your heel bone (the calcaneus) to the base of your toes, helping to support the arch and act as a shock absorber with every step. Plantar fasciitis — increasingly called plantar fasciopathy, or more broadly plantar heel pain — is an overload problem where that tissue, right at its attachment to the heel, is taking more strain than it can comfortably handle.
The “itis” ending suggests pure inflammation, but the current understanding is that it behaves more like an overload and gradual degenerative problem than a simple inflammatory one, with changes to the collagen structure of the fascia at a microscopic level (Morrissey et al., 2021). That's why the treatments that work best are the ones that load and strengthen the tissue over time, rather than ones that simply try to calm it down.
It's also useful to understand the biomechanics involved. With every step, your arch flattens slightly as weight comes through it, and the plantar fascia tightens to control that flattening — a mechanism sometimes called the “windlass mechanism,” because it works rather like winding a rope around a drum. Repeated, high, or poorly-distributed loading of this mechanism, thousands of times a day, is what eventually overwhelms the tissue's capacity to keep up.
Why does it happen?
It usually comes down to a mismatch between load and the tissue's capacity to cope with it. A sudden increase in walking or running distance, a lot of standing on hard floors, unsupportive or worn-out footwear, tight calf muscles and limited ankle movement all push up the demand on the fascia. Body weight and occupations that keep you on your feet all day — retail, nursing, hospitality, teaching — play a significant part too, and it's one of the most common causes of heel pain seen in general practice and physiotherapy clinics alike.
A higher arch or a very flat foot can both change how load travels through the fascia, and reduced ankle flexibility — particularly limited upward bend of the ankle (dorsiflexion) — forces the fascia to take on more of the shock-absorbing role that the calf and ankle would otherwise share. It's also more common in the 40–60 age group and in runners who have recently increased mileage, changed footwear, or returned to running after a break.
As with most overload injuries, it's rarely a single cause. More often it's a combination — new trainers, a busy fortnight on your feet at work, and calves that were already tight from reduced activity — that tips things over from coping to struggling.
What does it feel like?
The signature symptom is that sharp, stabbing pain under the inner heel with the first steps in the morning or after you've been sitting for a while — often described as walking on a stone bruise or glass. It tends to ease once you're warmed up and walking for a few minutes, then returns as a dull ache after prolonged standing or towards the end of the day, particularly after a long shift on your feet.
You can usually find a specific tender spot on the inside of the heel, just in front of where it meets the arch. The pain is typically one-sided, though it can affect both feet, especially in people whose job keeps them standing for long periods. If your symptoms include tingling, numbness, or a more diffuse, burning pain that spreads along the sole or into the toes, it may be something other than the fascia — which is exactly why an assessment, rather than self-diagnosis, is useful.
Conditions that can look similar
Heel pain has several possible sources, and telling them apart matters for getting the right treatment. A calcaneal stress fracture causes pain that is often more constant, worse with impact, and tender over a broader area of the heel bone rather than one specific spot — it's more common in runners who have sharply increased training load. Tarsal tunnel syndrome, caused by compression of a nerve near the ankle, tends to cause burning, tingling or numbness rather than a sharp mechanical pain, and often isn't limited to the first few steps of the day. Fat pad atrophy, more common in older adults, causes a deep, bruised-feeling pain across the whole heel with weight-bearing, without the classic first-step pattern. Achilles tendinopathy causes pain at the back of the heel rather than underneath it. And in people with inflammatory conditions such as ankylosing spondylitis or reactive arthritis, heel pain can be one of several joint and tendon symptoms, which is why a broader health picture is sometimes relevant. A clinical assessment helps make sure we're targeting the right structure from day one.
How we assess it
We'll ask about your footwear, your typical day — how much standing, walking or running it involves — and how your symptoms have changed over time. On examination, pressing on the inner edge of the heel bone usually pinpoints the pain precisely, and a simple manoeuvre — pulling the big toe upwards to tension the fascia through the windlass mechanism — often reproduces it, which is a reasonably reliable sign that the fascia is the source.
We'll also check your calf flexibility, ankle range of movement, and how your foot moves as you walk, because tightness or restricted movement anywhere in that chain loads the fascia more with every single step you take across the day. Importantly, part of a good assessment is ruling out the mimics discussed above, such as a stress fracture of the heel or a trapped nerve, which need a different management plan entirely. Imaging is not usually needed to confirm plantar fasciitis, but may be arranged if symptoms are atypical, prolonged, or not responding as expected to treatment.
How it's treated
Most people get better with the right conservative plan, and the mainstay is a combination of loading, stretching and short-term symptom relief while the tissue adapts. Calf and plantar-fascia stretches ease symptoms in the short term and are a sensible starting point, particularly first thing in the morning before weight-bearing. Alongside this, progressive strengthening — for example, heel raises performed slowly with the toes propped up on a rolled towel or the edge of a step, which specifically loads the fascia under tension — builds the tissue's tolerance over weeks and months and is the intervention with the strongest supporting evidence (DiGiovanni et al., 2003; NICE, 2024).
A typical progression starts with gentle, high-repetition calf raises to build tolerance without aggravating symptoms, before moving to slower, more heavily loaded single-leg variations as pain allows, and finally incorporating more dynamic, higher-impact loading for those returning to running or sport. Alongside this programme, hands-on treatment of the foot and calf, supportive taping to offload the fascia in the short term, and advice on footwear or a simple off-the-shelf insole can take the edge off symptoms while the tissue rebuilds its capacity. A night splint, which holds the ankle in a neutral position overnight, helps some people who are particularly stiff and sore first thing, by preventing the fascia from tightening up overnight the way it does in a naturally pointed sleeping position.
These add-ons are useful, but the progressive loading is what actually changes the story long-term. It does ask for patience — plantar heel pain often takes a few months to properly settle, sometimes six months or more in longer-standing cases — and the people who recover most smoothly are the ones who keep the loading programme going consistently and manage their standing and walking sensibly in the meantime, rather than swinging between total rest and doing too much on a good day.
How long does recovery usually take?
Timelines vary a lot between individuals, but it's helpful to have a rough sense of what to expect. Mild, recently-started cases often improve noticeably within four to six weeks of consistent stretching and loading. More established cases — particularly where symptoms have been present for several months before treatment starts — more commonly take three to six months to settle fully, and a small minority of long-standing cases can take up to a year. This isn't meant to be discouraging; it's meant to set realistic expectations, because people who understand the likely timeline tend to stay more consistent with their programme than those expecting a quick fix, and consistency is the single biggest predictor of a good outcome.
It's also worth knowing that improvement is rarely a straight line. Most people notice good and bad days, and a bad day after a longer walk or a busy shift at work doesn't mean the treatment isn't working — it usually just means that particular day's load outpaced the fascia's current capacity. Tracking your symptoms weekly, rather than day to day, gives a much clearer and less frustrating picture of your overall trend.
What you can do yourself
Sort out the basics first: supportive, well-cushioned shoes rather than flat or worn-out footwear, easing off (not necessarily stopping) the activities that clearly spike your pain, and a daily routine of calf and fascia stretches plus whatever strengthening programme you've been given. A gentle self-massage of the sole — rolling the arch slowly over a ball or a frozen water bottle for a few minutes — can take the edge off before those first difficult morning steps.
Use the 24-hour rule as your guide: some discomfort with activity is fine as long as it settles by the next day and doesn't leave your heel angrier the following morning. If it is worse the next day, that's your cue to ease back slightly rather than push through. Avoid walking barefoot on hard floors at home during the acute phase, and consider a supportive slipper or sandal even for short trips around the house.
When to seek help
It's worth getting assessed if the pain has lasted more than a few weeks, isn't improving with sensible self-care, or is changing the way you walk. Seek prompt review if you have heel pain that is severe at rest or wakes you at night, any numbness or tingling, or pain that followed a specific injury or sudden impact — these features point away from a simple, mechanical plantar fasciitis and towards something that needs a different kind of attention.
Frequently asked questions
Do I need to stop running or exercising completely? Usually not. Most people can keep some form of low-impact activity going while they work through a loading programme, adjusting volume based on the 24-hour rule rather than stopping altogether.
Are insoles or orthotics necessary? Not always, but an off-the-shelf supportive insole can meaningfully reduce symptoms for some people while the underlying capacity of the fascia is being rebuilt through exercise. Custom orthotics are occasionally useful in more resistant or biomechanically unusual cases.
What about injections or shockwave therapy? These are sometimes considered for cases that haven't responded to several months of well-run conservative treatment, but are not typically a first-line option, and work best when combined with, rather than instead of, an ongoing loading programme.
